What an educational evaluation is
This is an informational review — not a diagnosis, not a treatment plan, and not a commitment. A coordinator collects your metabolic history and forwards it to a physician for educational feedback.
What we typically ask for
- Recent A1c, fasting glucose, and (if available) C-peptide
- Height, weight, and BMI
- Current diabetes and weight medications, including GLP-1 or insulin
- Relevant conditions: hypertension, sleep apnea, fatty liver, PCOS
- Prior weight-loss or metabolic procedures, if any
What happens next
- You submit the intake form or call.
- A coordinator confirms your information.
- A physician reviews and shares educational feedback.
- If you'd like to go further, an in-person or video consult is scheduled.
